You are required to supply your own mask, for which fit-testing will be performed; if your company provides masks, you should be tested to that mask. Mask fitting is specific to the mask you are fitted to: it does not apply to any other mask. Please enter the make and model of the mask (eg. 3M-8210)*

Mask fit testing determines whether a seal is preserved during fit-testing. This testing is not about making a mask fit if it is too large or too small.

A commonly selected mask is the 3M-1870 mask for Medium to large faces and the 3M-1860 mask for small faces. The following link provides suggested locations where a mask might be available. The phone numbers of the companies are provided.

Please note: Wrap around the ear masks do not create a proper fit. Select a mask that has straps that are placed to the top of the head and the back of the neck. N95 masks that have breathing valves have been discouraged from use. If you are a student entering into a clinical setting you must be fit tested to the mask that will be worn in the clinical setting.

I will not hold Breath for Life responsible for medical health problems that may be encountered based on how I wear the mask after being certified. This includes changing the mask to a different style, make or model for which I was not tested to.*

The mask you will be wearing on a regular basis is the mask that you should be fit-tested to. If you decide to use a different mask at any time, you should be fit tested to that mask as well. This is essential to ensure that you are safe.

Breath for Life will do its best to accommodate your request and will confirm whether your requested time is available. Appointments are on a first come first served basis and the office will provide you with alternate dates and times if your requested date and time is unavailable. Communication will be by email.

I understand that mask fitting is about testing the seal of the mask to determine if the mask you selected fits properly. Breath for Life cannot make something fit if a seal is not preserved through the fit test.*

Breath for LIfe does not supply masks for the following reason: there are too many mask available and it is not feasible to stock every mask that is produced.

I understand that if a mask does not fit properly, which means that a seal was not maintained in the fit test, I will not be certified and will be required to supply a different mask.*

Breath for Life uses Bitrex (a bitter agent) to fit test.

I understand that when confirmed by email, that if I do not show for my fit test, there will be no refund of my payment. Any time greater than 30 minutes late will constitute a no-show. Any cancellations or reschedules of the testing will result in a loss of fees. I also understand that if I cannot be fit tested to the mask that I supplied, I will be given up to one week to provide another mask for fit testing.*
