Policies for Mask Fit Testing
- The mask that you are wanting to be fit-tested to must be the mask that you wear regularly in your workplace. The price paid is for one mask fit test only and not for multiple masks.
- I understand that if I do not show for my mask fit test there will be no refund of my payment.
- If you are more than 30 minutes late your lateness will be regarded as a no-show.
- Any cancellations or reschedules of the testing will result in a loss of fees: no credits or refunds will be offered.
- If the mask you are fit tested to does not maintain a seal during the fit testing, what does not fit represents a failed mask fit test and no certificate will be issued. If another mask is required for fit testing a new registration must be made.
- I agree that I will be clean shaven (if applicable), will not eat, drink , smoke (vape), chew gum or engage in any other activity not listed that could affect my ability to taste.
- With a successful fit-test a certificate will be issued.
- If you lose your certificate, you will be required to be fit-tested at full cost.
